Client Services Cost:
$300.00 initial appointment / $250.00 follow up appointments.
Currently all sliding scale & pro-bono spots are filled.
Professional Services Cost:
$160.00/hr Supervision & Business Coaching
Speaking fees vary, please inquire.
Insurance:
In Network: Aetna, Regence, Cigna and First Choice.
All other companies will consider us an out of network provider. Out of network, does not mean you aren’t covered. It means that the rate they will reimburse us is different and you may be responsible for a slightly higher portion than your regular co-pay. Prior to your first appointment, please contact your insurance carrier and ask the following questions:
- Does my plan cover nutrition therapy CPT codes of 97802 and 97803?
- Is there a limit to the number of visits I am allowed?
- Do I need a referral from a primary care doctor or therapist?
- Do I need a pre-authorization?
- Is my coverage limited to specific diagnoses?
